- List of all work experience and unemployment history for the past 10 years to present (NO GAPS, all years must be accounted for). Also include any military (DD214 form) and/or Peace Corps service.
- The following application materials: transcripts showing degree conferred, teaching certificates (current, expired, conditional), test scores
- Completed reference surveys from three current/most recent supervisors
