We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers. We are looking for full time School-Based Community Health Workers and you may be a good fit! Job Summary Community Health Workers (CHW's) play a crucial role in promoting and supporting the health of individuals, families, and communities. They are frontline public health workers who are trusted members of and/or have close understanding of the communities they serve. This relationship allows them to link patients and families with school, health, social and community services, to facilitate access to services, and to improve the quality and cultural competence of service delivery. CHWs build trusting relationships with their clients and help to conduct assessments, make referrals, offer health education, navigate complicated systems, and advocate for client's needs. The school-based CHW will primarily be working in the community within targeted Elementary Schools. CHW's will work closely with School administration and staff, Health Department personnel, medical providers, primary care teams, and other agencies to improve connections to health care and social services for families. CHWs will work to improve access to community resources that strengthen linkages among the medical home, school home, and neighborhood home environments. Requirements Qualifications: Education: High School Diploma, GED or HiSET Experience: Any combination of 2 years health/social services experience and/or education Verifiable good driving record and reliable transportation Background check Bilingual/bicultural in Spanish, Dari/Pashtu, Nepalese, Arabic, Swahili, and/or Portuguese
