Responsibilities
Patient Assessment: Conduct thorough assessments of patients' individual and family health needs, identifying potential risks and developing comprehensive care plans.
Care Plan Implementation: Deliver skilled nursing care in the home environment, including medication administration, wound care, disease management, and patient education.
Care Coordination: Collaborate with other members of the healthcare team to ensure coordinated and seamless care delivery.

Qualifications
Required:
Graduation from an accredited nursing program.
Washington Registered Nurse License upon hire.
National Provider BLS certification from the American Heart Association upon hire.
1 year of experience in acute care.
Preferred
Bachelor's Degree in nursing from an accredited school with public health preparation.
3 years of experience in home health, acute care, or a skilled nursing facility.
