Are you an experienced Registered Nurse looking for a rewarding remote opportunity? We are currently seeking dedicated RN Care Managers to work with Managed Care Plan members. This position is fully remote after a 2-3 week training period in Manhattan, NY.
Position Overview
As an RN Care Manager, you will provide clinical support and oversight to ensure our members receive safe and effective care in the community.
Key Responsibilities
Become proficient in Article 49 regulation.
Responsible for service authorizations, creating care plans, and coordinating services for high-risk populations.
Assess and coordinate hospitalization, skilled services, CHHA, and nursing home communication and planning.
Coordinate Benefits For Medicare, Medicaid, And Other Insurance.
Manage transitional care from acute to home, home to skilled nursing facility (SNF)/custodial, home to hospital, and acute to SNF/custodial.
Complete monthly call assessments and follow up on changes in medication, diagnosis, or significant events such as ER visits or falls.
Coordinate post-discharge services as needed.
Perform other duties as assigned.
Qualifications
Registered Nurse (BSN preferred) with a current, valid, and unencumbered license.
Experience as a NY-licensed RN in home care, acute care, sub-acute care, long-term care (LTC), or managed long-term care (MLTC).
Must maintain all state and federal compliance requirements for licensure and employment.
Bi-lingual and UAS certified candidates are preferred.
Must reside in downstate NY, Westchester, Long Island, or Northern New Jersey and be able to commute to Manhattan, NY for training and occasional in-office meetings.
