We are seeking experienced Registered Nurses for a Remote RN Care Manager position, working with Managed Care Plan members. This role is fully remote after 2-3 weeks of in-office training in Manhattan, NY. Qualified candidates must have a NYS-RN License and previous MLTC RN Care Management experience. Candidates must reside in downstate NY or NJ, be able to commute to Manhattan for training, and be available for in-office meetings if needed.
About The Role
As an RN Care Manager, you will provide clinical support and ensure members receive safe and effective care within the community. Your responsibilities will include creating care plans, coordinating services, and managing care transitions for high-risk populations.
Key Responsibilities
Become proficient in Article 49 regulation.
Manage service authorizations, develop care plans, and coordinate services across the care continuum.
Assess and coordinate hospitalizations, skilled services, CHHA, and nursing home communication and planning.
Manage coordination of benefits for Medicare, Medicaid, and other insurance.
Coordinate transitional care management from acute to home, home to skilled nursing facility (SNF)/custodial, and home to hospital.
Complete monthly call assessments and follow up on calls triggered by changes in medications, diagnosis, or events such as ER visits or falls.
Coordinate post-discharge services as needed.
Perform other duties as assigned.
Bilingual (Spanish, Cantonese, Mandarin) is a plus.
Qualifications
Registered Nurse (BSN preferred) with a current, valid, and unencumbered license.
Experience as a NY licensed RN in home care, acute, sub-acute, long-term care (LTC), or managed long-term care (MLTC).
Must meet all state and federal compliance requirements for licensure and employment.
Bilingual, UAS certified, and knowledge of care management preferred.
Must reside in downstate NY, Westchester, Long Island, or Northern New Jersey.
