Position Overview
We are currently seeking experienced Registered Nurses for a Remote RN Care Manager opportunity with a Managed Care Plan. This position is fully remote following 2-3 weeks of training in-office, located in Manhattan, NY. The ideal candidate will hold a NYS-RN License and possess previous MLTC RN Care Management experience. Candidates must reside in downstate NY or NJ, able to commute to Manhattan, NY for training, and available for occasional in-office meetings.
Responsibilities
Provide clinical support and oversight to ensure members receive safe and effective care in the community.
Become proficient in article 49 regulation.
Authorize services, develop care plans, and coordinate services across the care continuum for high-risk populations.
Coordinate Hospitalization / Skilled Services CHHA / Nursing Home Communication and Planning.
Manage coordination of benefits for Medicare, Medicaid, and other insurance.
Coordinate transitional care management (acute to home, home to SNF/custodial, etc.).
Conduct Monthly Call Assessments and follow up on triggered calls due to changes in medication, diagnosis, or events such as ER visits.
Coordinate post-discharge services as required.
Other duties as assigned; bilingual abilities (Spanish/Cantonese/Mandarin) are a plus.
Qualifications
RN (BSN preferred) with current, valid, and unencumbered license, certificate, or registration as required.
Experience as a NY licensed RN in home care, acute, sub-acute, long-term care (LTC), or managed long-term care (MLTCP).
Maintain compliance with all state and federal licensure and employment requirements.
Bi-lingual, UAS certified, and care management knowledge preferred.
Reside in Downstate NY, Westchester, Long Island, or Northern New Jersey.
Benefits
Competitive salary ranging from $100,000.00 to $115,000.00 per year, based on experience.
