New Concept Health Services is a locally-owned and family-oriented home health agency, providing individualized care throughout East Texas. We treat one another and our clients with Honesty, Respect, and Dignity. We are currently seeking qualified candidates for Registered Nurse (RN) part-time position, who has experience in Admissions and Records.

Qualifications:

• Current State Registered Nurse License
• Current Diver's License & Reliable Transportation
• 2 years Registered Nurse in clinical care
• 1 year previous Home Health Care
  
  

Responsibilities:

• Completes initial and ongoing assessments to identify the physical, psychosocial, and environment needs of home health patients/clients.
• Complete Outcome and Assessment Information Set (OASIS) in a timely manner
• Regularly re-evaluates the patient's/client's nursing needs
• Develops, initiates, and revises the plan of care in partnership with patient/client
• Communicates with all physicians involved in the plan of care and other health practitioners (as appropriate)
• Prepares clinical and progress notes
• Demonstrates commitment and professional growth by participating in agency-sponsored in-service programs
• Supervises, teaches and provides clinical direction to other nursing personnel
