We are seeking a compassionate Registered Nurse Case Manager to join our team. In this role, you will play a pivotal role in organizing and delivering hospice care, focusing on community health education and personalized patient care within their homes and communities.
Essential Job Functions/Responsibilities
Patient Care:
Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice needs.
Provide professional nursing care using the nursing process, including assessment, planning, implementation, and evaluation.
Develop individualized care plans based on nursing diagnoses, incorporating palliative care strategies and involving patients and families in the planning process.
Administer medications and treatments as prescribed, and assist with activities of daily living to promote patient comfort and independence.
Coordinate patient care as the Case Manager, ensuring continuity of care within an interdisciplinary team.
Communication
Maintain accurate clinical notes and communicate patient condition and care updates effectively.
Collaborate with physicians and community health providers to coordinate care plans.
Educate patients and families on self-care techniques, medications, and diet as prescribed.
Participate in interdisciplinary team meetings to ensure comprehensive patient care and emotional support.
Additional Duties
Participate in on-call duties as per policy and ensure availability of necessary equipment and services.
Supervise ancillary personnel and delegate responsibilities as needed.
Commit to professional growth through continuing education and participation in quality improvement activities.
Position Qualifications
Registered Nurse with current licensure in the state of California.
Graduate of an accredited school of nursing recognized by the National League for Nursing.
Current CPR certification.
Valid driver's license, auto insurance, and access to a reliable vehicle.
Minimum of one year of recent professional nursing experience; two years preferred, with at least one year in public health, home care, or hospice nursing.
Self-directed with excellent organizational and communication skills.
Ability to prioritize and manage workload independently.
Physical ability to perform required nursing tasks, including lifting, positioning, and transferring patients.
