Job Summary
As a Registered Nurse, you will play a crucial role in delivering compassionate nursing care to our patients. Working alongside an interdisciplinary team (IDT), you will evaluate, develop, and implement personalized care plans. Communication and coordination are key aspects of this role, ensuring seamless care for our patients.
Key Responsibilities
Develop and Manage Care Plans: Create and oversee individualized care plans in coordination with the IDT, adjusting as needed to meet patient and family needs.
Provide Compassionate Care: Perform clinical duties in accordance with competency checklists, policies, procedures, and standards of care.
Documentation: Maintain timely and accurate patient records using electronic charting.
Adhere to Policies: Implement and uphold hospice policies, procedures, and regulatory standards.
Foster Relationships: Go beyond the minimum care requirements to build meaningful connections with patients and their families.
Qualifications
Current CA RN license
At least 1 year of professional nursing experience within the last 2 years
Valid CA driver's license with proof of insurance
Proficient in computer skills (MS Word and Electronic Medical Record (EMR) systems)
Excellent observational, written, and verbal communication skills
Understanding of hospice philosophy and issues related to death and dying
CPR certification
Reliable transportation
