Summary:

The Remote Regional Care Management Director serves in the role of educator, advisor, consultant and driver in order to facilitate and implementation and enhancement of care management best practices in hospitals within a designated CHS division. Reports to the Vice President of Care Management.

Essential Duties and Responsibilities:

  • Identifies hospital need for intervention through a variety of activities, including but not limited to, data analysis, feedback from other Regional Directors, communication with hospital care management directors, hospital and or division leadership teams
  • Determines most appropriate and cost-effective means of intervention, strategically prioritizing site visits for the purpose of focused or comprehensive assessment or educational intervention.
  • Conducts effective, data driven care management assessments, identifying opportunities for implementation of best practices and creating goal oriented action plans in conjunction with the hospital care management director
  • Appropriately and effectively communicates opportunities for improvement and recommendations for change to impacted parties
  • Maintains high degree of accountability for assisting assigned hospitals in moving identified metrics to the established CHS goals
  • Assists senior leaders at the hospital to improve the leadership capabilities of existing Care Management leadership in assigned hospitals using evidence based approaches to leadership development and to select appropriate Care Management Directors, as needed

Qualifications:

  • Education/Experience:
    • RN with a minimum of a BSN and preferred Masters prepared in business, finance or management
    • Minimum of 3 years prior experience as a successful Acute Care Hospital Care Management Director
    • Certification within the field of Care Management (ACM or CCM) desired
    • Desired experience managing multiple sites and/or departments
    • Strong working knowledge of UR processes
  • Reasoning Ability: Ability to define problems, problem solve, analyze data, establishes facts, and draw valid conclusions. Must have the ability to interpret extensive amounts data and formulate plans for improvement. High level of responsibility and accountably and take ownership of CHS goals.
  • Computer Skills: Proficient in the use of Microsoft Office products or similar word processing, spreadsheet, and database tools

  • The Employee is required to read, review, prepare and analyze written data and figures, using a PC or similar, and should possess visual acuity.
  • The Employee may be required to occasionally climb, push, stand, walk, reach, grasp, kneel, stoop, and/or perform repetitive motions.
  • Must be able to travel 75-85% of the time
  • Must be able to be present at the corporate office for orientation and on a quarterly basis for ongoing self and program development
