The Recreational Therapist or Activities Coordinator role offers a unique opportunity at a new PACE (Program of All-Inclusive Care for the Elderly) program in West Baltimore within the framework of a supportive, innovative, and expert leadership team and company. Together with our coalition of strong partners within the community, the Recreational Therapist or Activities Coordinator will work closely with the Center Manager and participant care team to carry out our model of care, and have a transformative impact on a particularly under-served community in West Baltimore.

Provide the Right Amount of Medical Care: Elders often have many diagnoses, medications and specialists, involving complex care decisions. We help our participants understand their choices. We deliver thoughtful, appropriate and personalized care, doing what our participants want — nothing more and nothing less
Guide All the Care Provided: We always follow and guide our participants’ care — in their homes, at our center, in the hospital, and everywhere

To help us achieve this mission, we are looking for an experienced Recreational Therapist or Activities Coordinator who will develop and implement a comprehensive, outcomes-based recreation therapy program tailored to the goals as outlined in the care plan process that meet the needs of the frail elderly.
Main Responsibilities:
Leads the Recreation Therapy team in completing initial assessments, formulating the initial care plan, supporting 6 month and annual assessments and daily documentation as assigned by the Day Center Manager
Consistently monitors the diversity of the participants served and their interests/needs
Drives the Recreation Therapy Wellness calendar based upon those interests/needs as identified in the initial care plans, assessments and participant feedback
Ability to provide recreation therapy co-treatments and strategies such as Leisure Education and Leisure Counseling to other program departments
Provides training and programmatic feedback to Activity Coordinators and Recreation Therapy aides
Attends weekly team meetings
Leads and/or assists in large groups, small groups one-on-ones, and individual activities (exercise, arts and crafts, games, current events, sensory, etc.)
Assists in the organizing of special events
Attend staff meetings and participant care planning on a regular basis
Qualifications:

Graduate of a Recreational Therapy program for the Recreational Therapist position
For Activities Coordinator, must possess, as a minimum, a GED or high school diploma and Activities Coordinator experience
Experience working with a frail or elderly population preferred
Knowledge of the principles and techniques involved in relating to frail elderly persons required
Must submit to and pass a pre-employment drug/alcohol screening and criminal background check
Must have a current Driver’s License and driving record must be in compliance with Company policy. Must maintain current automobile insurance
Up-to-date vaccinations, including COVID vaccine, required
