Scope

The Public Health Planner reports to the Program Manager and works closely with the Director of the Carlton-Cook-Lake-St. Louis Community Health Board (CHB), CHB staff, and local public health partners.


Location

The successful candidate must live within, or very near, Carlton, Cook, Lake, or St. Louis County (Minnesota) or be willing to relocate.


Compensation + Benefits

$27 – $29 / hour plus benefits (health, dental, and life insurance, paid time off, retirement benefits, and employee assistance program).


Schedule

This is a full-time position with expected hours of Monday – Friday, 8:00 AM – 4:30 PM. Ability to flex hours to accommodate for personal schedules is available (e.g., workday starting at 8:30 AM to accommodate childcare drop-off).

Purpose

This role will focus on carrying out the current Community Health Improvement Plan (CHIP) across the four-county region, including sharing communication with the public related to priority health areas. Duties related to the CHIP include: preparing reports and presentations, leading work planning meetings with local public health partners, researching public health strategies, documenting steps taken/lessons learned, evaluating CHIP effectiveness, and a focus on strategies that support health equity.


This is a three-year grant funded position.


Essential Job Duties and Functions Include

  • Create and maintain working relationships with local public health staff and community-based partners, like schools or nonprofits.
  • Work with the Public Health Communications Coordinator to prepare content for posting on social media.
  • Meet deadlines while working in a hybrid environment. Hybrid environment means employees have the option to work from a home office or the CHB office in Duluth. Some in-person meetings and trainings will be required.
  • Manage multiple projects, while coordinating the details of the project plan with various partners.
  • Compile project reports upon request.
  • Program Planning: Use data to support planning efforts related to the CHB’s established Community Health Improvement Plan (CHIP). Lead the implementation of the CHIP, including addressing the goals, objectives, and strategies for improving health priorities written into the plan. Research existing data sources to support data-informed decision making. Collect additional data for planning and evaluation efforts, as needed. The 2024 priority health areas include: Mental Health & Wellbeing, Substance Misuse, and Healthy Eating / Active Living (HEAL).
  • Evaluation: Evaluate the effectiveness of our CHIP strategies. Support local public health with evaluation efforts related to their local CHIPs upon request. Use evaluation findings to improve future processes and strategies.
  • Perform any other duties as assigned by CHB director.

  • Change Maker: The grant supporting this position challenges the public health workforce to do things different than they’ve been done before. Strong candidates are excited about systems change, consider themselves a life-long learner, and are adaptable to change. Interest in learning about a wide-variety of public health programs to support work across program areas.
  • Team Oriented: Strong interpersonal skills and ability to build relationships with CHB staff and local public health partners. Has excellent listening skills and can communicate effectively with project partners across a variety of channels (email, phone, virtual meetings, in-person meetings, etc.).
  • Computer Savvy: Ability to use Microsoft programs like Word, Excel, PowerPoint, SharePoint, and Adobe. Comfort using/learning Outlook for email and work calendar. Ability to use, or willingness to learn and use, Canva and social media platforms (Facebook, Instagram, etc.).

Qualifications

  • A bachelor’s degree.
  • Experience working successfully with community partners.
  • Ability to work both independently and as part of a team.
  • Previous experience, or interest in learning about, creating social media calendars, posting content, and evaluating effectiveness.
  • Live within, or very near, Carlton, Cook, Lake, or St. Louis County.
  • Experience in the field of public health, government, and/or nonprofit is preferred.

Additional Requirements

Travel. This position requires occasional travel for projects and training. Access to reliable transportation for occasional travel (e.g., 1-2 times per month) throughout the 4-county region is needed.
