At Dakota County, Public Health Nurses work 8:00 a.m. to 4:30 p.m., Monday through Friday. We offer flexibility with your time off requests, reimbursement for mileage traveled for work, free continuing education, and free parking!

The FAMILY HEALTH PUBLIC HEALTH NURSE provides home visits to antepartum, postpartum, and parenting families in Dakota County using an evidence-based home visiting model. The position also provides education and health consultation to various community groups.

Location & Schedule

This hybrid position offers a mix of telework, in-person work at our Northern Service Center in West St. Paul MN, and fieldwork throughout Dakota County. Work is performed Monday through Friday during regular business hours (8:00 a.m. to 4:30 p.m.).

What You’ll Do

As a Family Health Public Health Nurse, you will work with families using an evidence-based home visiting model. You will conduct holistic assessments of family needs in a culturally sensitive manner, develop and implement nursing interventions, monitor outcomes, and complete thorough documentation in an electronic health record. You will determine client eligibility for county programs and provide linkages to community resources. You will provide health consultation and education on individual, family, and community levels through direct client service and collaboration with partners. You will also participate in emergency response activities as outlined in the department emergency response plan.

Minimum Qualifications

Need to Have Qualifications

  • Bachelor’s Degree in Nursing AND
  • Current licensure as a Registered Nurse in Minnesota AND
  • Current licensure or qualifications to register as a Public Health Nurse in Minnesota or ability to obtain before start date AND
  • Valid driver’s license AND
  • CPR Certification

Nice To Have Qualifications

  • Professional nursing experience in acute care or community-based settings
  • Experience in health-related areas such as women, children, parenting, or lactation
  • Experience and training in an evidence-based home visiting model
  • Fluency in Spanish or Somali

Duties & Responsibilities

  • Determine eligibility for public health programs and manage service requests for department and community services.
  • Conduct a holistic assessment of client/family needs in a culturally sensitive manner by using independent nursing assessment skills and professional judgment.
  • Develop and implement nursing intervention plans based on nursing diagnosis to insure achievement of timely client outcomes.
  • Perform case management functions such as monitoring the quality of services, identifying service gaps, and ensuring services are delivered within authorized budget.
  • Direct paraprofessional staff in delivery of client services.
  • Provide health consultation and education on individual, and community levels through direct client service, collaboration with other health care providers, and by participating in community task forces, to plan for and ensure service coordination with community agencies.
  • Complete prescribed forms and documents in accordance with department policies and procedures and professional standards.
  • Work to advance inclusion, diversity, equity, and access in collaboration with colleagues and Dakota County residents
  • Perform public health emergency response functions as trained and assigned within the department’s emergency response plan.
  • Perform others duties as assigned by supervisor.

WORK ENVIRONMENT: Work is performed in a hybrid model with work via telework, within a County office building and on client home visits. Travel is required. Exposure to hazardous physical conditions such as environmental conditions of unsanitary homes and property, unsafe neighborhoods, challenging and potentially violent and unpredictable clients and family members, pets, and animals; exposure to atmospheric conditions such as chemicals, odors, dusts, gases, poor ventilation, and cigarette smoke; exposure to hazardous materials such as chemicals, blood, and other body fluids; extreme temperatures; inadequate lighting; intense noise, and work space restricting movement may occur on visits to client's homes. Lifting requirement of up to 50 pounds on an occasional basis. May require lifting of up to 100 pounds on an infrequent basis. Equipment used may include, but is not limited to, computer, printer, telephone, typewriter, calculator, copy and fax machines, TV/ VCR, video camera, nursing supplies and equipment including scales, syringes, hazardous waste, masks, blood pressure equipment and car seats.
