Overview
Reporting to the Program Manager, the Program Coordinator supports the Program for Older Adults with Breast Cancer in the Breast Oncology Center, Susan F. Smith Center for Women’s Cancers. This program provides personalized support to older patients throughout their breast cancer journey, from diagnosis through treatment into survivorship. The Program Coordinator acts as the primary point of contact for older patients and their caregivers, conducts initial clinical intakes, offers patient navigation and communicates with the oncology care team. Work closely with the Lead Program Coordinator in day-to-day operations and program coordination, and with the Program Manager and Medical Director as well on tasks related to program development, implementation, and evaluation. This position is based at the Chestnut Hill campus is primarily an on-site, in-clinic position.

Responsibilities
- Initiate contact with newly diagnosed older patients with breast cancer and with existing patients referred/self-referred to the program, maintain ongoing check-ins and follow-ups with patients and referral contacts, document all interactions in the program database and Epic medical records when indicated. Related tasks may include:
- Meet patients in person, via phone or Zoom to provide an introduction to the program.
- Conduct comprehensive clinical assessments with new patients to understand their needs and priorities.
- Manage referrals as indicated within Dana-Farber, e.g. social work counseling and support groups, nutrition, Zakim integrative therapies, financial services, peer mentorship, survivorship, community-based services.
- Provide patient navigation and case management services to assist patients with accessing supportive care services and resources, and to address barriers to services and resources.
- Maintain an introductory packet for patients new to the program as well as educational and informational print/digital resources curated for this patient population.
- Assist with program communications and public relations, e.g. program webpage, Emma patient emails and newsletters, identifying patient and program stories for Dana-Farber communications venues.
- Assist with coordination of patient programs, including educational forums and ongoing support groups dedicated to older adults.
- Maintain data entry in the program database, which tracks enrolled patients, referrals, services provided, and communications with patients and providers.
- Support expansion of the program’s scope, such as new initiatives or new sites, over time.
- Act as a liaison to BOC research, clinical trials and other research projects related to cancer and aging.
- Collaborate with the Lead Program Coordinator and Program Manager on administrative activities related to protocols, reporting and program evaluation.
- BA/BS required. Advanced degree in public health, social work or health care administration a plus.
- Two years related professional experience required. Experience working directly with patients or in health care a plus.
- Previous experience developing and managing a database preferred.
- Excellent interpersonal skills to communicate effectively with patients with sensitivity, compassion and patience, and writing skills to communite with providers, professionally and timely
- Ability to self-direct and work independently with trong organizational skills and attention to detail, and excellent follow-up of tasks and communications
- Ability to use/learn multiple computer applications required, including Microsoft Office, MGB hospital systems, Epic and MyEmma communication tool.
