Description The Evergreen Crossing Primary Care Clinic is a primary care clinic that operates at the site within Central City Concern's Recuperative Care Program (RCP). RCP is an innovative service that provides immediate access to emergency housing, primary care, and intensive case management support for individuals who have medical support needs while experiencing homelessness. The service partners with Portland area hospitals and clinics, in concert with CCC's patient-centered medical home, to help clients/patients receive the right level of care and support for their medical and social needs. The position works collaboratively as a member of a multi-disciplinary team providing medical and at the Evergreen Crossing Primary Care Clinic. This position provides primary, urgent, and acute care to a population of low-income adults with a broad range of physical, behavioral, and social conditions. This position is for a Medical Doctor, Naturopathic Doctor, Doctor of Osteopathy, Family Nurse Practitioner or Physician Assistant to work in CCC's Primary Care department with a primary assignment at the Blackburn Center. This position provides primary, urgent, and acute care to a population of low-income adults with a broad range of physical, behavioral, and social conditions. This position also provides Medication Supported Recovery services through CCC's M3 program, our low-barrier buprenorphine/naloxone program, and supports patients receiving Hepatitis C treatment through our pharmacy-based Hep C Treatment Program. A strong background in mental health and addiction medicine and ability to meet the training requirements for buprenorphine prescribing are strongly preferred. Experience working with homeless and underserved populations is strongly preferred. Successful applicants will understand the value of harm reduction and low-barrier buprenorphine prescribing and will be flexible and responsive to highly vulnerable patients. Demonstrated skills in primary care medicine and licensure to practice medicine in Oregon are required of all applicants. Join our team at Central City Concern! We are looking for a Primary Care Provider at our Evergreen Crossing Primary Care Clinic! Schedule: Monday-Friday 8:00am-5:00pm MINIMUM QUALIFICATIONS: NP/PA

  • Graduate of an accredited Nurse Practitioner Program or Physician Assistant program required of all Nurse Practitioner or Physician Assistant applicants.
  • If candidate is a Nurse Practitioner, graduate of an accredited school of nursing and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure.
  • Current Advanced Practice Registered Nurse (APRN) Licensure with National certification or Licensed to practice as a Physician's Assistant in Oregon, with at least 2 years' experience. MINIMUM QUALIFICATIONS: MD/DO/ND
  • Board eligibility or certification in Internal Medicine or Family Practice.
  • Graduation from a CNME-accredited, four-year Naturopathic Medical program.
  • Must have completed a CNME-certified primary care residency program.
  • Licensed naturopathic physician in the State of Oregon.
  • Maintain an Active DEA Certification. MINIMUM QUALIFICATIONS: ALL APPLICANTS
  • A strong background in mental health and addiction medicine is preferred, as well as experience working with homeless and underserved populations.
  • Demonstrated skills in primary care medicine with a minimum of 2 years' experience preferred.
  • Must meet applicable training requirements for Buprenorphine prescribing.
  • Spanish language fluency preferred.
  • Must meet CCC privileging requirements as required by FTCA.
  • Must have full prescriptive powers in Oregon with a DEA number.
  • Current BLS certification prior to start.
  • Available to work during standard clinic hours, occasional evenings until 8 pm, and Saturdays 9 am - 1 pm 1 - 2 / month.

  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity. ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Determine and formulate health histories and perform physical exams.
  • Order, interpret and evaluate diagnostic tests to identify and assess patients' clinical problems.
  • Based on available clinical data, work cooperatively with patients and care team to formulate and implement person-centered care plan.
  • Conduct on-going health care and clinical management of stable, chronically ill patients. Provide provider services in Office Based Opioid Treatment program, including substance use treatment with buprenorphine and other medications
  • Support pharmacy-based Hepatitis C program and offer medical visits to support clients receiving treatment.
  • Collaborate with integrated care team to provide most cost-effective and high-quality care.
  • Determine and manage minor and/or acute illnesses and initiates treatment.
  • Conduct initial care for emergency situations such as illness or injury and initiate arrangements for continuing care.
  • Prescribe medication or other forms of treatment as necessary.
  • Assess need for and perform clinical procedures, including joint injections, incision and drainage of abscesses, skin biopsies, and toenail removal.
  • Keep appropriate records.
  • Participate in and, at times, lead Provider Education Sessions on a variety of clinical topics.
  • Participate in compliance, quality assurance and quality improvement activities as directed.
  • Collaborate with care team to ensure optimal performance on team- and clinic-based performance metrics.
  • Work as a member of the Evergreen Crossing Primary Care Clinic multi-disciplinary team to deliver comprehensive, client centered and trauma informed integrated services.
  • Actively participate in the integration of services at the Evergreen Crossing Primary Care Clinic and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
  • Proactively identify opportunities for improvement within Evergreen Crossing Primary Care Clinic workflows and logistics. Participate in review of Evergreen Crossing Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
  • Build understanding of and appreciation for diversity among Evergreen Crossing Primary Care Clinic clients and support their connection to culturally responsive and culturally specific services as requested.
  • Attend all mandatory CCC trainings in a timely manner.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  • Perform other duties as assigned, including occasional work at other CCC clinical sites and/or off-site at community events. SKILLS AND ABILITIES:

  • Ability to consider impacts of systems of oppression, structural racism, and individual bias on client outcomes.
  • Ability to display non-judgmental attitude.
  • Sufficient manual dexterity and physical ability to perform assigned tasks.
  • Ability to manage time and meet deadlines.
  • Knowledge of de-escalation methods or willingness to be trained in these methods.
