Position Overview

The Practice Coach provides practice transformation support to OCHIN’s members and customers to help them establish and reach clinical, operational, and/or quality improvement goals; adopt best practices; implement innovations; and improve patient outcomes. The Practice Coach is a member of the Practice Transformation in the Clinical Operations department.

We are seeking to fill two Practice Coach positions. The goal of this recruitment is to fill the position of Practice Coach. However, we encourage candidates who meet the minimum qualifications for the Associate Practice Coach ($70,214 - $87,767) and the Sr. Practice Coach ($81,669 - $106,170), to apply, as the department may consider a more junior or senior candidate.

Essential Duties

  • Provide direct coaching support to clinical practice teams, both in-person and remotely
  • Facilitate member team participation in research, operational, and quality improvement projects
  • Assist member teams in obtaining and using data to drive change and improve outcomes
  • Use change management and quality improvement principles to provide feedback and recommendations to member clinics for improving clinical and organizational processes
  • Support member team adoption of OCHIN’s technology solutions (e.g., Healthy Planet, Reporting Workbench) to drive practice transformation along the building blocks of primary care
  • Facilitate meetings with and between clinics to identify facilitators and barriers to change and share clinical and operational best practices
  • Build and maintain relationships with internal and external stakeholders and continuously seek opportunities to improve OCHIN’s systems and processes through practice coaching services
  • Manage multiple projects related to clinical improvement simultaneously and actively participate in OCHIN cross-departmental project teams as needed (e.g., Research, Innovation)

  • Serve as lead coach on a grant activity


Job Duties – Sr. Practice Coach

  • Utilize a broad perspective to interpret issues, recommend solutions, and support team members in resolving complex problems
  • Provide proactive input to the Manager on design and implementation of team processes.


Requirements

All Levels

  • Experience in a telecommuter role required, including use of remote technologies (e.g. Zoom)
  • Demonstrated experience communicating to a wide range of roles/stakeholders, both writing and oral presentation; ability to identify/monitor tasks and timelines and work on multiple projects simultaneously, and demonstrated experience with electronic documentation and file management, including documenting health center engagement and maintaining status reports
  • Experience working with electronic health records desirable. Previous experience with FQHC/Safety Net clinics is desired, as well as working knowledge of clinical quality metrics such as UDS, HEDIS, NCQA, OR NQF type metrics
  • Demonstrated coaching and/or team building to support health center change agents apply evidence-based data-driven solutions
  • Demonstrated experience supporting individual clinicians, care teams, and clinic leaders with implementing practice changes in a clinical setting using principles of Motivational Interviewing or Human Centered Design; practices of Model for Improvement or Lean; and frameworks such as PCMH or the Building Blocks of High Performing Primary Care.
  • Excellent computer skills, especially Microsoft applications Word, Excel, PowerPoint, Teams and SharePoint; comfort with exploring and learning new applications or databases.
  • Self-motivated, preferably with strength in anticipating and resolving problems, and demonstrated problem solving skills, preferably with progressive management of increasingly complex tasks and strong process understanding of project lifecycle
  • Ability to create welcoming, structured spaces for meeting and collaboration, virtually and in-person, including creating realistic agendas and creative social connections (icebreakers), and coordinating stakeholders across disciplines and technical assistance needs.
  • Ability to travel to urban and rural locations.

  • Minimum of one (1) year of experience supporting health care teams in similar or relevant role
  • Bachelor’s degree in healthcare (e.g., public health, health promotion); communications or change management, or a related field strongly preferred


Requirements – Practice Coach

  • Minimum of three (3) years of experience supporting health care teams in similar or relevant role
  • Bachelor’s degree in healthcare (e.g., public health, health promotion); communications or change management, or a related field strongly preferred


Requirements – Sr. Practice Coach

  • Minimum of five (5) years of experience supporting health care teams in similar or relevant role
  • Bachelor’s degree in healthcare (e.g., public health, health promotion); communications or change management, or a related field strongly preferred
  • Demonstrated strength in anticipating and resolving problems including where little to no precedent may exist, strong process understanding of project lifecycle
  • Substantial applied practice facilitation – 5+ years of experience in the field


Base Pay Overview

The typical offer range for this role is minimum to midpoint, ($77,235 - $96,544) with the midpoint representing the average pay in a national market scope for this position. Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will consider a wide range of factors directly relevant to this position, including, but not limited to, skills, knowledge, training, responsibility, and experience, as well as internal equity and alignment with market data.

Work Location and Travel Requirements

OCHIN is a 100% remote organization with no physical corporate office location. Employees work remotely from home and many of our positions also support our member organizations on-site for new software installations. Nationwide travel is determined based on OCHIN business needs. Please inquire during the interview process about travel requirements for this position.
