The Social Worker is responsible for assessing and meeting the needs of patients and families, providing psychosocial support, crisis intervention, and discharge planning from the emergency room. They are an integral member of the care team, assisting patients and families in the psychosocial aspects of care. Interventions may include connecting patients and families to necessary resources and supports in the community or helping a patient to expand and strengthen their network of social supports. This position will support staff and patients in the Emergency Room in creating a safe and timely discharge plan, by providing information, resources, and post-discharge education.

Job Essentials
Emergency department experience and crisis intervention experience preferred
Must have excellent communication skills
Demonstration of professional excellence and excellent organizational skills
Current driver's license required and access to a vehicle
Night and weekend hours required
