Ready to join? We are looking for a part- time Physical Therapist to join our team. The Physical Therapist is responsible for the evaluation, development and implementation of treatment programs in collaboration with physician orders.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Evaluates patient and gathers appropriate information to determine accurate clinical diagnoses and treatment plan.
  • Establishes appropriate, functional and objective goals based upon clinical examination.
  • Devise detailed treatment and care plans for patients.
  • Provides exceptional patient care and service.
  • The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships.
  • Cultivate relationships with patients and the community.
  • Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care.
  • Maintain patient charts, clinical logs and checklists necessary for compliance.
  • Maintains patient confidentiality with regard to medical records per company policies and procedures and assures compliance with all HIPAA guidelines.
  • Communicates well in a professional setting.
  • Perform additional duties as required.

Gait Training

  • Same day post op gait training, exercise instruction, passive ROM
  • Patients will be seen in surgical centers for all treatments (including total shoulder, total knee, total hip, ACL’s and fusions)
  • Travel required to Ambulatory Surgery Centers: Rush Oak Brook Surgery Center, Kendall Pointe, AMSURG, Munster Surgical Suites, Great Lakes, South Suburban, Rush Surgi Center and Lincoln Park Surgery Center.

Education And/or Experience

  • Bachelor’s, Masters or Doctorate Degree from an accredited Physical Therapy
  • COVID and Flu vaccination required to be credentialed in surgery centers.
  • CPR Certified.
  • One year of outpatient experience, preferred.

Certificates, Licenses, Registrations

  • Current State of Illinois Physical Therapy license with eventual application for Indiana license.
