The Physical Therapist – Provides outpatient quality physical therapy within the patient’s own home and acts as a liaison for physician practice to meet the therapy needs of patients. The position is best suited for an outgoing and self-motivated therapist. The position offers a high level of flexibility as well as autonomy to take ownership of a practice and offers a new standard of post-operative care. The position will afford the therapist to spend one on one time with patients and also the latitude to help grow a new service line.

Responsibilities
One on one in home therapy for post-operative patients
Devise and execute detailed treatment plans for post-operative patients
The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships.
The ability to work independently, providing respectful and empathetic patient care at all times.
Cultivate relationships with patients and the community.
Provide continuous communication with referring physicians and patient families to ensure a consistent level of care.
Maintain patient charts, clinical logs and checklists necessary for compliance.
Set up and monitor home exercise program.
Provide education specific to diagnosis and symptom management needs.
Recognizes and responds to priorities in patient care showing initiative and sound judgment.
Demonstrates willingness to adapt to change in workload and assists with training/orienting others.
Stays abreast of and complies with all state and federal laws including HIPAA, ADA, OSHA, and FLSA.
Other projects as assigned.
Education And/or Experience
Bachelor’s Degree from an accredited Physical Therapy Program.
Minimum one-year experience in outpatient setting preferred.
Certificates, License, Registrations
Current State of Illinois Physical Therapy license.
Current CPR Certification.
BCLS or ACLS.
Valid Driver’s License, reliable transportation and required liability insurance to perform home visit
