Responsibilities:
Assess each patient and develop an appropriate treatment plan with measurable goals and time frame
Prepare and submit clinical and progress summaries based on identified and attained goals
Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
Open/Supervise cases for LPTA's to do follow-ups
Qualifications:
Active License of Physical Therapy in the state of Virginia
Prior Homecare experience (preferred)
EMR system: Kinnser Wellsky (preferred)
