Position Summary

Ensures patients have access to medications by:

  • processing medication renewal requests and prior authorization
  • assist patient obtain medications from medication assistance programs
  • provide outreach effort to those clients who are at risk of falling out of care by scheduling appointments and provide prescription refills as needed

Essential Duties & Functions

  • Serves as liaison between Legacy client and the provider/pharmacy to minimize time between medication needs are met.
  • Monitor all incoming requests for prescription renewals (i.e. from electronic renewal requests, pharmacies, and patients) and respond as per protocol within 48 hours of receipt.
  • Identifies requests that fall outside prescription renewal guidelines and seeks assistance from pharmacist and providers, when appropriate.
  • Demonstrates ability to understand provider’s plan for appropriate patient’s follow-up.
  • Identifies patients (via report) and takes appropriate actions to ensure patients’ continued accessibility to prescriptions. These actions include but are not limited to making an appointment, providing refill, communicating with provider, and obtain lab orders when necessary.
  • Provide last reminder to patient with medications that will soon return-to-stock.
  • Document medication non-adherence in the EMR via “Phone Notes” for patients who have frequent incidences of RTS for chronic medications.
  • Monitor for and respond to in-coming requests for prior authorization.
  • Document prior authorization process per procedure and update status as appropriate.
  • Stays abreast of available Pharma’s Patient Assistant Program (PAP) which provides medication access to those who have no other means to pay.
  • Monitor for and respond to providers’ referrals for PAP assistance within 24 hours.
  • Identify appropriate PAP programs applicable to patient’s needs.
  • Maintains accurate and updated documentation for all related work, in Legacy’s EMR and Legacy Pharmacy system.
  • Performs other duties as assigned.

Required Education, Certifications, Licenses, & Training (minimum Requirements)

  • High School Diploma or equivalent required
  • Certification as a Pharmacy Technician, or LVN
  • Must maintain BLS /CPR

Required Years Of Experience (minimum Requirements)

  • Minimal 5 years’ experience as a certified pharmacy technician, or LVN

Required Knowledge, Skills, And/or Abilities (minimum Requirements)

  • Demonstrates written and verbal communication skills, courteous and clear
  • Demonstrates excellent customer service skills
  • Understanding of medication prescriptive process
  • Computer software knowledge
  • Develops, understands, and demonstrates proficiency in navigating electronic medical records.

Desirable Knowledge, Skills, And/or Abilities

  • Spanish speaking preferred
