We are seeking a proactive and independent Nurse Supervisor to join our team at a leading healthcare services provider. As Nurse Supervisor, you will play a pivotal role in shaping our organizational culture and foundation while driving growth and satisfaction among our valued clients and referral sources.
Key Responsibilities
Cultivate and nurture relationships with community referral sources to enhance brand awareness and foster long-term partnerships.
Ensure the satisfaction of referral sources to sustain ongoing business growth and reputation.
Represent our organization at networking events and community gatherings to promote our services effectively.
Handle inquiries from potential clients, providing comprehensive information, and scheduling assessments or services as needed.
Conduct nursing assessments with prospective clients to develop personalized Care Plans.
Collaborate closely with physicians, case managers, discharge planners, pharmacies, durable medical equipment vendors, and other interdisciplinary partners to coordinate optimal client care.
Maintain effective communication within the team to ensure alignment and high-quality service delivery.
Provide thorough care management, including ongoing assessment of client conditions, oversight of clinical services, and support and education for assigned clinicians.
Qualifications
Valid RN license with X years of nursing experience (include specific requirements as needed).
Proven ability to build and maintain professional relationships.
Excellent communication skills, both verbal and written.
Strong organizational and time-management abilities.
Ability to work independently and as part of a collaborative team.
