Responsibilities
Cultivating Connections: Develop and nurture relationships with community referral sources to enhance brand visibility and foster trust.
Client Satisfaction: Strive for excellence in customer satisfaction by understanding and meeting the needs of our referral sources, fostering long-term partnerships.
Brand Representation: Serve as a proud ambassador for Care at networking events, community gatherings, and other relevant platforms.
Client Engagement: Responsively address prospective client inquiries, providing comprehensive information and facilitating assessments and service commencement.
Care Planning: Conduct thorough nursing assessments with prospective clients, collaborating with them to develop personalized Care Plans tailored to their unique needs.
Collaborative Coordination: Work closely with medical professionals, case managers, and other stakeholders to ensure seamless coordination of care and services for our clients.
Team Collaboration: Communicate effectively with fellow team members to ensure optimal client-provider matching and overall service quality.
Client Care Management: Provide attentive care management for all clients, conducting regular reassessments, overseeing clinical services, and offering support and education to assigned clinicians.
Qualifications
Bachelor's degree in Nursing (BSN) preferred.
Active RN license.
Proven experience in healthcare management or related field.
Strong interpersonal skills and ability to build rapport with diverse stakeholders.
Excellent organizational and communication abilities.
Commitment to delivering high-quality care and fostering a positive client experience.
