Nurse Case Manager (RN)

Overview

Ansible Government Solutions, LLC (Ansible) is seeking a Nurse Case Manager to to support the Durham VA Medical Center located at 508 Fulton St. Durham, NC 27705. Working hours are Mon-Fri, 8:00am-4:30pm. Weekend coverage may be required on a rotational basis. Competitive packages are available for qualified candidates.

Responsibilities

• Coordinates all systems/services required for an organized, multidisciplinary, patient-centered care team approach
• Helps ensure quality, cost-effective care for the identified patient
• Manages the course of treatment of patients, coordinating care with physicians, nurses and other staff
• Ensures quality patient outcomes are achieved within established time frames and with efficient utilization of resources
• Conducts initial and ongoing assessments
• Determines and manages outcomes
• Ensures continuity of care through discharge planning
• Functions as a contact person for patient, family, health care team members, community resources and employees as necessary
• Ensures adherence to departmental policies and procedures
  
  

Qualifications

• This position requires U.S. Citizenship for DOE contract security clearance
• Shall have a health professional-related degree, e.g. BSN
• Active state RN license
• Certification as a Case Manager (CCM) is highly desired
• 3 years of case management experience
  
  

All candidates must be able to:

• Sit, stand, walk, lift, squat, bend, twist, and reach above shoulders during the work shift
• Lift up to 50 lbs from floor to waist
• Lift up to 20 lbs
• Carry up to 40 lbs a reasonable distance
• Push/pull with 30 lbs of force
