Role Summary
Are you a compassionate and experienced nurse looking to lead and support a dedicated team from the comfort of your home? Join us as a Chronic Care Management (CCM) Team Lead! In this remote role, you'll act as a key resource for our nurse team, providing guidance and support when the team manager is unavailable. You will assist with patient triage, offer training during orientation, and serve as a preceptor to new nurses managing patients with complex health conditions. This position is based out of California but is open to applicants in the PST time zone.
Key Responsibilities
As a CCM Team Lead, your daily responsibilities will include:
Patient Triage: Assess and triage patient calls, addressing both routine and emergent clinical needs.
Care Plan Management: Identify patient needs and create, manage, and update individualized care plans.
Preventative Assessments: Conduct telephonic assessments to screen for new issues, reassess patients, and evaluate progress toward care goals.
Symptom Evaluation: Monitor and evaluate changes in patient-reported symptoms, conducting additional triage and screening as necessary.
Referral Support: Facilitate referrals to primary care providers or hospitals when clinically indicated.
Patient Education: Educate patients and their families on treatment options, including costs, efficacy, and potential side effects.
Administrative Tasks: Complete phone and chart audits as directed by the team manager.
Job Requirements
To succeed in this role, you should possess the following qualifications and skills:
Licensing: Active, unrestricted RN license in the state(s) of practice. A multi-state license is required.
Experience: Minimum of two years of related clinical experience, with a preference for telephonic care management experience.
Technical Skills: Proficiency in Microsoft Office programs and the ability to document patient assessments telephonically.
Time Management: Excellent organizational skills, flexibility, and the ability to work independently.
Empathy and Ethics: High degree of empathy and compassion, coupled with strong ethical standards.
Analytical Skills: Ability to analyze patient claims data, draw conclusions, and take timely action based on that information.
