Role Summary

Are you a compassionate and experienced nurse looking to take on a leadership role? We're seeking a dedicated CCM Team Lead to join our team. In this role, you'll serve as a key resource for the nurse team, stepping in when the team manager is unavailable. Your expertise in patient triage, especially for emergent clinical needs, will be invaluable. You will also play a crucial role in training new staff during orientation and mentoring new nurses who care for patients with complex illnesses, recovery from traumatic events, or managing multiple co-morbidities. Comfort with tasks such as phone and chart audits is essential. This is a REMOTE position based out of California, and we welcome applicants from the PST time zone.

Daily Responsibilities

Your daily responsibilities will include, but are not limited to, the following:

Patient Assessment and Triage: Perform thorough assessments and triage of patient calls, identifying clinical needs and developing appropriate care plans.

Care Plan Management: Manage patient care plans, addressing issues as they arise and providing continuous support.

Preventative Assessments: Conduct regular telephonic assessments to screen for new issues, reassess the patient, update care plans, and evaluate progress towards goals.

Symptom Evaluation: Monitor changes in patient-reported symptoms, conducting additional triage and screenings to determine necessary next steps.

Referrals: Refer patients to their primary care provider or hospital when clinically necessary.

Patient and Family Education: Educate patients and their families on treatment options, including cost, efficacy, and potential side effects.

Job Requirements

We are looking for candidates who possess the following qualifications:

Excellent Time Management: Ability to manage time effectively, be flexible, and stay organized while working independently.

Active RN License: Must have an active, unrestricted RN license in the state(s) of practice, with a multi-state license required.

Clinical Experience: Minimum of two (2) years of related clinical experience, with telephonic care management experience preferred.

Technical Proficiency: Proficient in Microsoft Office programs and capable of documenting patient assessments telephonically.

Empathy and Ethics: Highest degree of empathy and compassion, with excellent ethical standards.

Analytical Skills: Ability to analyze patient claims data, draw conclusions, and act on that information promptly.
