Nurse Care Manager - Remote | WFH

We are seeking a compassionate and experienced Nurse Team Lead to join our team. As a vital member of our Care Coordination Management (CCM) team, you will play a pivotal role in supporting our nurses and patients. This remote position, based in California and open to applicants in the PST time zone, offers an exciting opportunity to make a difference in the lives of those managing complex illnesses or recovering from traumatic clinical events.

Key Responsibilities

As a CCM Team Lead, your daily responsibilities will include:

Providing invaluable support to our nurse team, particularly when the team manager is unavailable.

Conducting patient triage, including addressing emergent clinical needs with confidence and efficiency.

Mentoring and training new nurses during orientation, serving as a knowledgeable preceptor to ensure seamless patient care.

Conducting phone and chart audits as directed by the team manager, ensuring adherence to quality standards.

Daily Role Responsibilities

Your essential functions in this role include:

Performing thorough assessments and triage of patient calls to identify clinical needs promptly.

Developing and implementing tailored care plans to support each patient's unique requirements.

Managing issues identified by the patient care plan, ensuring comprehensive and effective care delivery.

Conducting proactive telephonic assessments to screen for potential issues, update care plans, and monitor progress towards established goals.

Responding promptly to changes in patient-reported symptoms, conducting further triage and screening as necessary.

Facilitating referrals to primary care providers or hospitals when clinically warranted.

Educating patients and their families on treatment options, including associated costs, efficacy, and potential side effects.

Job Requirements

To excel in this role, you'll need:

Exceptional time management, flexibility, and organizational skills, coupled with the ability to work autonomously.

An active, unrestricted RN license in the state(s) of practice, with a Multi-State license being a requirement.

A minimum of two (2) years of relevant clinical experience, ideally with telephonic care management experience.

Proficiency in Microsoft Office programs, enabling efficient documentation while conducting telephonic patient assessments.

A profound sense of empathy and compassion, coupled with unwavering ethical standards.

The ability to analyze patient claims data, derive insights, and take appropriate action in a timely manner.
