Purpose: (General description and summary)

The Mental Health Clinician (MHC) provides services including behavioral health counseling, case management, skills training, and care coordination to individuals in the Rehabilitation/Outpatient Program at New Narrative’s Mill Park Center. The MHC is responsible for developing, implementing and monitoring services to individuals based on their individual goals. Focus of this position will include stabilization in the community, ensuring program participants have access to services that support their overall wellness, advocating that individual rights and dignities be upheld, empowering individuals to achieve their individual goals. Interventions take place both in the office and in community settings.

Accountabilities: (Responsibilities of the job)

Case Management and Behavioral Health Counseling

  • Develop treatment alliance with participants by meeting for therapeutic services
  • Services may be rendered both in the clinic and in the community
  • Assess the strengths and service needs of each participant
  • Develop individual service plans in partnership with the participant, which includes measurable objectives
  • Assist participant in connecting to resources and services needed to achieve goals set in individual service and support plans
  • Provide individual behavioral health counseling utilizing evidence based practices that address mental health symptoms
  • Facilitate structured group therapy
  • Develop relationships with providers and stakeholders to facilitate service provision.
  • Assist with coordination and attendance of medical appointments, social supports and legal requirements as needed
  • Utilize appropriate assessments when indicated in treatment
  • Apply feedback informed treatment model.

Crisis Intervention

  • Work proactively with participant to avoid use of crisis services
  • Complete and maintain crisis alerts for coordination with county crisis services for participants
  • Coordinate and document hospital/inpatient service transitions and program transfers when necessary, including care coordination with hospital staff to support discharges.
  • Effectively utilize crisis de-escalation skills to support all individuals, including milieu support as Clinician of the Day
  • Complete and document risk assessments and safety plans
  • Ensure documentation is completed same-day for each person who may be at increased risk of crisis service interaction
  • Complete critical incident reporting form within 24 hours when indicated
  • Carry after hours phone on an alternating basis for care coordination support

Administration:

  • Ensure clinical charts are complete and in compliance with federal, state and county rules, including service plans and LSI/LOCUS
  • Complete documentation within 72 hours of services being rendered through EHR (or sooner if crisis risk indicates need for same day completion).
  • Complete supportive documentation in timeframes indicated by OARs
  • Utilize feedback-informed treatment tools and review regularly
  • Participate in one-on-one supervision bi-weekly to review interventions, documentation, and professional goal setting
  • Attend and positively participate in required meetings, including weekly staff meetings and care coordination meetings with LMPs.

Utilization Reviews

  • Provide information as requested by the Quality Improvement Coordinator
  • Complete all documentation and observe timelines to ensure that charts are in compliance
  • Participate in supervision as needed to review utilization

Other General Expectations:

  • Attend and positive participate in required meetings
  • Adhere to any applicable ethics codes
  • Follow all New Narrative policies, procedures, and protocols
  • Adhere to regular work schedule as set by direct supervisor
  • Follow through on all directives and instructions
  • Maintain current Oregon Drivers’ License
  • Maintain CPR and First Aid Certificate
  • Demonstrate effective organizational and time management skills
  • Pursue ongoing development of skills and competencies as required by individuals licensing bodies and as required to maintain competency in the role

Education/Certificates/Licenses/Registration:

  • Qualified Mental Health Professional (QMHP) eligibility with a Master’s Degree in social work, counseling, or related field is required
  • CADC credentials or interest in obtaining a CADC encouraged
  • Ability to obtain Oregon driver’s license within 30 days of hire

Experience/Qualifications:

  • One year experience working in community mental health providing case management and/or behavior health treatment, particularly with individuals with severe and persistent mental illness preferred
  • Multi-lingual preferred
  • Clinicians from various backgrounds, experiences, and identities support an effective clinical team. New Narrative values and works to intentionally practice Diversity, Equity, and Inclusion principles and integrate feedback from the Peer Movement

Mental Requirements:

  • Extensive mental activities required. Examples to include but not limited to the use of educated judgment, communicate thought clearly, maintain calm demeanor in crisis situations, be an empathetic and effective listener, and flexible analytical and interpreting skills
  • Ability and willingness to triage priority with tasks and delegate as needed to maintain quality services rendered in high caseload volume setting
  • Ability to motivate others and remain consistently enthusiastic

New Narrative outpatient operations are an essential part of New Narrative’s overall continuum of service and supports. As a member of the clinical team, the MHC will help to insure best practices, positive clinical outcomes, and quality of services.
