Primary Responsibilities/Accountabilities:

  • The Social Worker MSW provides services to patients/residents and their families/caregivers to meet their social and emotional needs as they relate to their medical diagnoses and concerns.
  • They collaborate with the multidisciplinary team to ensure that patients receive treatment, recovery, and safe and coordinated transition from one care environment to another.

Qualifications:

  • Master's Degree Degree as a Social Worker from an accredited School of Social Work.

Preferred:

  • Certification in Oncology Social Worker (OSW) in Oncology Setting
  • American Case Management Certification (ACM) in Acute Care Setting
  • 2 years Medical Social Work experience.
