Pulls charts for appointments and all requests for charts in a timely manner, including from Medical Records File room, old charts in boxes off-site and in inactive records
Prepare folders and maintain records of new patients
Transports patient charts for daily appointments to designated areas including prep area, team rooms, file room, charting area and other areas as required
Retrieves completed charts from designated pick up locations for filing in Medical Records Department
Processes and files patient records and related correspondence in the Records Department in accordance with established protocols
Assures that each providers has initialed and dated or signed all paperwork before filed into chart
Searches chart files to detect and correct filing errors
Identifies and combines duplicate charts into one record
Monitors and maintains old and inactive patient medical records
Purges old record and prepares for off-site storage in accordance with established protocols
Answers telephone, takes messages, and/or directs requests for records and related patient information to appropriate staff members, as required
Operate computer to enter and retrieve data and type correspondence and reports
Provides clerical and other office services to the Medical Records Department which includes filing, typing, faxing and photocopying
Process authorized request for copies of medical records to outside agencies including patients, physician offices/medical facilities, disability determination, legal summons in accordance with established protocols
Demonstrates a caring and helpful attitude when interacting with patients, vendors, and fellow employees
Strives to build cooperative partnerships, by treating each person both inside and outside of the organization as a customer
Ability to establish and maintain effective working relationships with other clinical personnel
Handles confidential information with tact and discretion on a need-to-know basis
Complies with confidentiality requirements as detailed in HIPAA regulations and all other state and local rules, regulations and guidelines
Performs other related duties as assigned
Requirements
High School Diploma or equivalent
Knowledge of Windows based program (Excel, Word)
Detail oriented
Must be able to alphabetize and file accurately
Type words and numbers quickly and accurately
Must comply with HIPAA confidentiality standards when accessing or communicating patient information
While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel and reach with hands and arms. The employee is frequently required to stand; walk; sit; climb or balance; stoop, kneel, crouch, and talk or hear. The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to adjust focus.
