Summary
This position will work exclusively with the Primary Care practices in Sault Ste. Marie. The coordinator will be responsible for patient education, patient triage, medication refills and assisting in a clinical capacity as needed in all Primary Care locations. The coordinator will also assist all Primary Care practices with inbasket tasks as well as communicate with patients via MyChart
Responsibilities
(75%)*Triage patient calls and report lab/test results to patients; will schedule ancillary services and make referrals to other physicians and/or patient education as needed; Communicate with patients via MyChart; Manage medication refills across all Primary Care locations.
(15%) * Acts as a quality and clinical support staff leader within the clinic to assure high quality patient care. Works gaps in care reports in Epic to ensure completion of required health maintenance measures.
(10%) * Assist providers with direct patient care as needed. OTHER DUTIES AND
RESPONSIBILITIES: Participates in annual Skills Day; manages POCT stations as requested by leadership. Works collaboratively with LPN nurse educator to train new clinical staff members. Provides for the specific needs of the population served according to department standards and policies/procedures as evidenced by observation, documentation and peer feedback. Understands necessary computer functions in the office setting: Electronic Medical Record, Dictation systems, and any other necessary programs to assist providers, patients and staff. Room patients during office visits and perform and record all vital signs on patient. Verify patient demographic information and maintain clinical documentation as outlines in MyMichigan Medical Group policies and procedures.
Other duties as assigned by the Practice Manager.

Certifications and Licensures
BLS - Basic Life Support Required
Equivalent Experience - Within 1 month of hire or transfer.
RN - Registered Nurse Required

Required Education
Education: Associates Degree
Comments: BSN is preferred. Experience in complex care management and/or patient education.
Other Information
EXPERICNE TRAINING AND SKILLS: Working knowledge of the organization’s Lawson, EMR computer programs preferred or knowledge of similar software. PC software knowledge must include Excel and Word. Knowledge of medical office policies, practices and procedures as normally acquired through three to five years working in the healthcare system or similar setting. Excellent interpersonal skills needed to communicate successfully with individuals and interact with people at all levels to communicate concepts in a clear and understandable manner. to stressful situations, including those involving public contact, as well as, trauma, grief and death.
