Focusing on outpatient medicine, continuity of care, health maintenance, and disease prevention
Demonstrates both autonomy and proficiency in performance of comprehensive health assessments and communicate Plan of Care with Patients and/or their families.
Proficient in diagnosing, treating and managing the Medicare Advantage population with multiple comorbidities.
Effectively communicates with all members of the care team and facilitates a medically appropriate and effective plan of care to avoid unnecessary duplication of testing, and fragmentation of care.
Observes the scope of practice in the state in which he or she is licensed and working in.
Schedule Requirement
This is a FULL-TIME or PART - TIME position.
Monday to Friday 8:00 am - 5:00 pm
Eligibility & Qualifications
Ability to use Electronic Medical Record (EMR) system to document patient encounters.
Knowledge of ICD-10 Coding and STAR measure attainments
Effective communication and interpersonal skills
Demonstrated ability to problem-solve complex and multifaceted situations.
Strong organizational, and task prioritization skills
Education/Experience
Medical Doctorate (M.D.) or Doctor of Osteopathic Medicine (D.O.); Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
Medical License in the state of Texas
DEA License
Up-to-date CPR Certification (BLS, ACLS, and PALS)
