As a Medical Case Manager, you'll play a pivotal role in the recovery journey of injured workers and their families. Your responsibilities will involve close collaboration with patients, their families, medical providers, and our team to facilitate recovery. This role requires significant local travel to manage a caseload of workers' compensation cases within a designated area.
Key Responsibilities
Case Management: Provide comprehensive medical case management to individuals through both in-person and telephonic communication with patients, physicians, healthcare providers, employers, and other stakeholders.
Treatment Planning: Use your medical and nursing expertise to discuss current and alternative treatment plans with physicians, ensuring they are appropriate, necessary, and cost-effective.
Patient Assessment: Evaluate patient treatment plans and progress through ongoing assessment, planning, implementation, and evaluation.
Collaborative Visits: Attend appointments with doctors, other providers, and in some cases, attorneys, to coordinate care. Participate in hospital and long-term care facility discharge planning to develop effective long-term care strategies.
Home Visits: Conduct initial home visits for patient evaluations and implement necessary care, including the negotiation of durable medical equipment and nursing services.
Travel Requirements: Regular travel is required based on the needs and injuries of the injured worker, with local travel up to approximately 60% of the work week/month and occasional overnight travel.
Skills And Qualifications
Communication and Multi-tasking: Effective communication and multi-tasking skills in a high-volume, fast-paced, team-oriented environment.
Clinical Background: Experience as a Registered Nurse (RN) or Medical Case Manager is ideal. A clinical background in orthopedics, neurology, or rehabilitation is preferred.
Interpersonal Skills: Ability to meet and interact with patients, physicians, healthcare providers, attorneys, advisors/clients, and coworkers.
Cost Containment: Background in cost containment, such as utilization review or managed care, is helpful.
Organizational Skills: Strong interpersonal, time management, and organizational skills.
Technical Proficiency: Proficiency with MS Office, including Excel spreadsheets.
Independence and Teamwork: Ability to work both independently and within a team environment.
Education And Experience
Nursing Education: Graduate of an accredited school of nursing.
Licensure: Current RN licensure in the state of operation.
Certifications: Certification as a CCM, CIRS, or other case management certifications are preferred.
Driver's License: Valid driver's license, reliable transportation, and ability to travel to assigned locations.
Compensation
Pay Range: $29.25 - $43.72 per hour.
Salary Determination: Pay rates are established based on federal, state, and local minimum wage requirements, geographic location, job-related skills, experience, qualifications, internal employee equity, and market conditions. Adjustments to pay rates may occur based on the assessment during the interview process.
