The role of the Withdrawal Management “Detox” Case Manager is to support patients during their stay and help them with discharge plans and connection to resources that further their goals. The Case Manager works with a dynamic multidisciplinary team in a fast-paced environment The Case Manager communicates and collaborates with outside agencies and referents to make the best plan possible for each patient. This role requires an independent, creative person, eager to multitask and do something different every day! Support takes the form of teamwork, weekly team meeting, and regular supervision meetings.
ESSENTIAL JOB FUNCTIONS
Treatment Planning and Transitions
Review the treatment options appropriate for the patient's needs, characteristics, goals, and financial resources
Meet with each patient individually
Work appropriately with the patient to recognize and encourage skills consistent with progress toward treatment goals
Keep accurate documentation of intervention/support activities. Maintain records on patient progress.
Provide referral services to assure that patients access a variety of services and benefits, including mental health services, shelter/housing, treatment services, medical services
Participate in admission and discharge planning for any patient needing hospitalization or subacute level of care.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to Fora policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for electronic medical record access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving Fora services by Fora or an outside provider to the Fora Quality Management Department, as well as to your supervisor or their designee
Counseling and Education
Maintain a patient-centered, trauma-informed approach to working with patients while taking into consideration the integrity and safety of the withdrawal management milieu
Utilize nonviolent communication, verbal de-escalation, and motivational interviewing
Focus on building skills with patients based on their assessed level of care and continuing care needs
Understands basic principles of harm reduction, peer-delivered services, readiness to change, and the ASAM criteria for treatment placement, continuation, and transition
Understand and learn to address the case management needs of patients who struggle with activities of daily living and develop/maintain housing referral sources for those patients
ADDITIONAL JOB RESPONSIBILITIES
Relationship/trust building/loving kindness with patients and staff
Participate in training opportunities to advance skills and knowledge in mental health and addiction
Ability to learn how to respond to mental health crises in a clinically and culturally appropriate way
Perform special projects and other duties as assigned
OTHER REQUIREMENTS
For employees with a history of substance use disorder or identifying as a person in recovery, Fora Health requires certification of a minimum of two years’ continuous sobriety
Successful completion of pre-employment drug test
Successful completion of Tuberculin test and/or evaluation with negative results or documented evidence of non-communicability
DHS criminal background approval
SUPERVISORY FUNCTIONS
This position has no supervisory responsibility.
QUALIFICATIONS
Education and Experience:
1 year experience working in a health care or substance use treatment field required
CADC-I is required / CADA-II is preferred
Associate’s degree or higher is preferred
Knowledge, Skills, and Abilities:
Medical Case Manager will work to establish professional and supportive relationships.
High School Diploma or GED required
Flexible work schedule required
Will be required to carry an agency cell phone for work use
Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit
Oregon Administrative Rules prohibit Substance Use Disorder program staff from having a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act within the previous 3 years
Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects
Must be able to climb stairs several times a day
Must pass a DMV background check and be designated an “acceptable” driver as outlined in Fora’s Policy. Must pass an initial Annual Drivers training and then pass an annual Recertification training
Must adhere to agency’s non-discrimination policies, including but not limited to, non-discrimination, safety, drug and alcohol-free workplace and harassment-free workplace
Ability to effectively interact with co-workers and patients who have diverse ethnic or cultural backgrounds, religious views, political affiliations, lifestyles and sexual orientation and treat each person with respect and dignity

WORKING CONDITIONS
This position works both in an indoor and outdoor environment
Possible exposure to communicable diseases which can encompass a variety of illnesses and infections, including tuberculosis, Hepatitis A, B, and C, and HIV
Possible exposure to upset, angry, severely traumatized, disabled or emotionally disturbed adults, adolescents, and families
Exposure to potentially hazardous cleaning chemicals, personal protective equipment provided.
HOURS AND PAY
This is a full time position earning $27.14-28.80 per hour (depending upon experience). This position is non-exempt and eligible for overtime compensation.
LOCATION
This position is an on-site position at our SE Cherry Blossom location in Portland, Oregon.
