We are excited to invite you to join the Acclaim Rehabilitation and Nursing Center team as an MDS Assessor. In this role, you will play a pivotal part in orchestrating the MDS process and providing invaluable support to our esteemed care planning team.
Salary: Up to $90k
Full Time: Weekdays

Your Responsibilities as an MDS Assessor:
In this role, you will be entrusted with critical duties to ensure the highest standards of care for our sub-acute and long-term residents:
Assess and appraise the caliber of care being administered to long-term residents.
Initiate comprehensive interviews and evaluations of incoming residents.
Help develop meticulously structured care plans encompassing both short-term and long-term objectives for advancement.
Spearhead interactions with multidisciplinary treatment teams, caregivers, and families through productive meetings.
Participate in Nursing management team meetings and seamlessly interact with all levels of Nursing staff.
Conduct enlightening in-service education sessions on MDS, CAAs, and Triggers.
Adhere diligently to the MDS/Care Planning schedule.
Complete accurate MDS assessments
Monitor and audit timely completion and accuracy of care plans, clinical and MDS assessments of healthcare team members
Attend IDCP meetings
Complete Medicare certifications and PDPM forms
Oversee NOMNC, ABN, and exhaust letter compliance
Monitor and analyze quality indicators
What We're Looking For:
Our ideal candidate is armed with the following qualifications and attributes:
Possess a valid New Jersey RN license
Bring to the table a rich history of at least 2 years in a Skilled Nursing Facility.
Demonstrate a comprehensive understanding of MDS, Medicare, and the care-planning process, illustrating your expertise.
