Our Client is seeking Licensed Social Workers to provide services in the Chicagoland area.
Summary Of This Opportunity
Screen, assess, diagnose, create treatment plans, and conduct therapeutic interventions with clients
Maintain regular, ongoing contact with clients, caregivers, and physicians.
Conduct reassessments and monitor clients' progress.
Complete and submit timely paperwork and reports.
Review and modify care plans as directed by the client/caregiver during the reassessment and obtain the client's signature of approval.
Maintain regular, ongoing contact with clients, caregivers, physicians, and service providers listed on the service plan through home visits and phone calls.
Required Experience And Education For Licensed Social Worker
Must have a Professional Educator License (PEL) in the state of Illinois with an endorsement in Social Work.
