The Licensed Clinical Social Worker (LCSW) collaborates closely with healthcare providers, nurse case managers, pharmacists, and other team members to develop Member Action Plans (MAPs) for individuals facing social, psychological, and environmental barriers impacting their healthcare access and transitions within care. This role receives referrals network-wide, identifying high-risk members for population health management programs. The LCSW connects members with community-based resources and embodies core values such as Imagination, Partnership, Forthrightness, Humility, Courage, and Relentlessness.
Primary Responsibilities
As an LCSW, your responsibilities will include:
Identifying and enrolling members who would benefit from program participation through assessments and risk screenings.
Advocating for members, families, and caregivers within the healthcare system.
Conducting assessments to evaluate members' functioning levels.
Providing interventions including counseling, family support, and clinical referrals for mental health disorders and addiction treatment.
Coordinating care needs across the continuum by facilitating provider contacts and care team appointments.
Engaging timely and cost-effective community resources to optimize member outcomes in medical and social health determinants.
Making appropriate referrals to community resources.
Facilitating direct communication between the care team, members, caregivers, and relevant providers to enhance outcomes.
Applying knowledge of external and internal regulatory requirements in case management.
Educating individuals and families on strategies and skills to overcome challenges.
General Duties
The LCSW Will
Work with the care team to identify high-risk patients suitable for population health management programs.
Collaborate across departments to align clinical objectives and knowledge for effective care delivery.
Assist in developing Member Action Plans (MAPs) for high-risk patients facing social, psychological, or environmental barriers.

Qualifications
The ideal candidate should possess:
Active New York State Licensed Clinical Social Work (LCSW) certification.
Two to five years of experience in community-based organizations (CBOs), provider offices, or government entities, providing referrals to community resources, benefit guidance, and mental health counseling to underserved populations.
Entrepreneurial mindset with a commitment to addressing healthcare disparities in Black and Brown communities.
Excellent verbal, written communication, and presentation skills.
Proficiency in Microsoft Office applications and willingness to learn Electronic Medical Record (EMR) platforms.
Strong relationship-building skills with clinical and business leaders and external stakeholders.
Strong ethical standards and a collaborative approach to team success.
Remote position based in California.
Compensation
Salary: $80,000 - $85,000 annually
