We are seeking a compassionate and professional Health Coach and Consultant to join our team. In this role, you will play a vital part in helping our members overcome psychosocial barriers to managing their health conditions. Your responsibilities will include:
Providing expert guidance to members and program staff on addressing psychosocial challenges related to health management.
Making proactive outbound calls to members to offer health coaching and consultation, supporting their journey to better health.
Assisting participants in changing self-efficacy behaviors and finding and accessing intrapersonal, family, and community resources to facilitate health management.
Consulting with staff members on effective methods and approaches to help participants recognize and overcome psychosocial barriers to improved health.
Requirements
A Master's degree in Social Work or a related behavioral health field.
A minimum of 3 years of clinical experience, or an equivalent combination of education and experience.
Current Indiana license as a Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC).
Relevant experience in a call center or healthcare-specific professional setting is preferred.
Competencies / Knowledge / Skills
Proficiency with Microsoft Office applications, including Outlook, Word, and Excel.
Sensitivity to and experience working within different cultures.
Strong interpersonal skills and the ability to work independently and as part of a team.
Ability to identify problems and opportunities, and effectively communicate them to management.
Growing knowledge of local, state, and federal healthcare laws and regulations, as well as company policies related to case management practices.
Demonstrated compassion, support, and collaboration with members and their families.
Comfort with asking pertinent questions to gather necessary information.
Ability to thrive in a fast-paced environment.
Commitment to ethical conduct and promoting ethical behavior.
Skill in developing positive relationships with all stakeholders.
Awareness of community and state support resources.
Strong organizational skills, attention to detail, and conflict resolution abilities.
Ability to maintain composure and professionalism during times of high emotional stress.
A strong sense of confidentiality and acting in the best interest of the organization and its members.
