We are seeking a dedicated Health Coach to join our team. In this role, you will play a crucial part in supporting members and program staff by providing guidance on overcoming psychosocial barriers to managing health conditions. You will engage with members through outbound telephone calls, offering health coaching and consultations aimed at empowering them to adopt healthier behaviors. Additionally, you will assist participants in identifying and accessing personal, familial, and community resources that facilitate improved health management. Your expertise will also be pivotal in advising staff on effective strategies to address psychosocial barriers, contributing to enhanced health outcomes for our members.
Key Responsibilities
Provide guidance to members and program staff on psychosocial barriers to health management.
Conduct outbound telephone calls to deliver health coaching and consultations.
Assist participants in enhancing self-efficacy and accessing relevant resources.
Offer consultation to staff members on effective approaches to overcoming psychosocial barriers.
Requirements
Master's degree in Social Work or related behavioral health field.
Minimum of 3 years of clinical experience, or equivalent combination of education and experience.
Current clinical licensure in Indiana as a Clinical Social Worker (LCSW) or LMHC.
Preferably, experience in call center or healthcare settings.
Required Competencies
Proficiency in Microsoft Office (Outlook, Word, Excel).
Cultural sensitivity and experience working across diverse populations.
Strong interpersonal skills with the ability to work independently and collaboratively.
Problem-solving Abilities And Effective Communication Skills With Management.
Developing knowledge of healthcare laws, regulations, and company policies.
Compassionate and collaborative approach with members and families.
Comfortable asking pertinent questions and maintaining professionalism in emotionally charged situations.
Organizational skills with attention to detail and conflict resolution abilities.
Commitment to ethical conduct and building positive stakeholder relationships.
Awareness of local and state support resources and their utilization.
