What You Will Do
The Intake Coordinator is a non-exempt position? responsible for admitting and coordinating initial services to the Hub clients.
Ensure clients are admitted into the Hub on the same day as arrival for service.
Complete all intake paperwork comprehensively and make referrals to additional services, as necessary.
Run reports in electronic health record system as required.
Participate in trainings and team meetings.
Maintain contact with clients through routine calls and follow-ups.
Complete client satisfaction surveys and track data for outcomes.
Perform all other duties as assigned.

What You Will Need
Bachelor's degree in a human services related field preferred
or Bachelor’s degree from an accredited college/university and two (2) years’ experience working in a human services related field.
Must have a valid drivers' license and auto insurance coverage.
