Job Summary
The Intake Coordinator interviews new Empowering Minds Resource Center (EMRC) clients to collect client data and to complete comprehensive assessments.
Duties
Makes virtual and or home visits to new clients for intake interviews
Collects patient's physical, familial, and psychiatric background data -Completes comprehensive assessments
Explains agency policies, procedures and processes, and program rules and regulations
Ensures all paperwork is completed correctly and promptly and meets EMRC’s compliance standards
Ensures all patient consent forms are signed
Requirements
Excellent organizational skills
Must be detailed Oriented
Ability to professionally handle confidential information and comply with HIPAA regulations
Excellent verbal and written communication skills
Excellent interpersonal/human relations skills
Background investigation (state and federal)
Own and operate a tablet to document and electronically submit necessary forms
A car to travel to new clients’ homes for intake interviews
Minimum Qualifications
Bachelor's degree in a Human Services field
2 years of field/direct service work in mental health/social work
Job Type: Part-time
Pay: $27,000.00 - $35,000.00 per year
Expected hours: 20.00 per week
