We are seeking a meticulous and detail-oriented Medical Coder to join our team. In this role, you will be responsible for accurately coding and abstracting both inpatient and outpatient medical records in accordance with ICD-10-CM and CPT-4 guidelines. Your primary focus will be ensuring that all codes are assigned with 95% accuracy and in compliance with regulatory standards. You will collaborate closely with physicians to clarify documentation and resolve any coding discrepancies to support timely billing processes.
Responsibilities
Code all inpatient records within 5 days of discharge, ensuring 95% accuracy and following ICD-10-CM guidelines. Perform interim coding when necessary.
Code all outpatient records by the third day post-discharge, including Ancillary and Wound Care services, with 95% accuracy using ICD-10-CM and CPT-4 guidelines.
Collaborate with physicians to clarify documentation when code assignments are ambiguous or inadequate.
Perform necessary investigations to ensure seamless account transition into the billing system. Notify Director of any potential delays.
Utilize HMS for accurate coding and ensure all required information is documented to facilitate clean claims processing.
Monitor billing error reports to ensure timely and accurate account processing. Report any anomalies to the Director.
Abstract designated physician information fields to maintain accurate physician index reports.
Requirements
High School diploma or equivalent; CIC, CPC, CCS Coding Certification, or RHIT degree required.
Prior Experience In Inpatient Rehab Coding Is Essential.
Proficiency in ICD-10-CM coding, medical terminology, anatomy, physiology, pharmacology, and laboratory terminology.
Ability to stay updated on coding changes and regulations.
Excellent communication, organization, and numerical filing skills.
Strong computer literacy, including data entry proficiency.
Ability to maintain confidentiality, exercise initiative, and work independently.
Positive attitude and ability to collaborate effectively with physicians and hospital staff.
