We are currently looking for a Hospice RN Clinical Manager to join our team. This position would be responsible for coordinating total care for patients with life threatening and life limiting illnesses, and their families and managing the day to day operations of the Hospice Team.

Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Provides overall operational responsibility for the Hospice program and coordination of patient care
  • Manages a caseload of hospice patients as needed
  • Conducts community outreach activities to improve community access to hospice care
  • Monitors quality and appropriateness of services provided to ensure patient satisfaction
  • Adheres to agency infection control and safety policies
  • Supervises staff in conjunction with the organization’s managerial and leadership practices
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications

  • RN with BSN; oncology/hospice and palliative care certification preferred
  • 2-3 years experience with community health oncology/hospice nursing preferred
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills

Working Conditions/Physical Demand: Working indoors/outdoors with clinical equipment and use of phone and computer; client/customer contact
