Assess the psychosocial status of patients and families/caregivers in relation to the patient's terminal illness and environment, and communicate findings to the registered nurse and interdisciplinary team members.
Maintain clinical records for all patients referred to social work services.
Educate and orientate new admissions.
Provide information and referral services to patients, families/caregivers, addressing practical and environmental needs.
Offer information to patients, families/caregivers, and community agencies.
Act as a liaison between patients, families/caregivers, and community agencies.
Foster collaborative relationships with organization personnel to enhance patient care.
Contribute to the development of individualized care plans and attend interdisciplinary meetings regularly.
Assist physicians and team members in understanding significant social and emotional factors related to health challenges and end-of-life concerns.
Required Qualifications
Master's Degree in Social Work or Counseling Psychology.
Minimum of 1 year of experience as a Social Worker or Counselor.
Preferred Qualifications
Minimum of 1 year of experience in hospice care.
Bilingual proficiency.
Compensation
Per-Diem Rate: $65 - $80/hour
