Home Health Physical Therapist Assistant (PTA) - $5,000 Sign on Bonus

Sierra Home Health & Hospice is seeking a Physical Therapist Assistant (PTA) for its comprehensive Home Health Program. The PTA will provide skilled therapy services under the direction of a licensed Physical Therapist in the comfort of patients' homes, and must have a willingness to travel to Truth or Consequences, in addition to Las Cruces. This is an exciting opportunity for a PTA professional who values independence and enjoys establishing rapport with patients and their families. With branches in both Las Cruces and T or C, Sierra provides comprehensive Home Care and Hospice Programs and is staffed with experienced clinicians to ensure patients receive the highest quality of care.

Responsibilities

• Perform patients' evaluations and obtain necessary clinical data
• Develop and implement individualized treatment plans based on clinical observations and objectives established by a licensed Physical Therapist
• Administer treatment programs as prescribed by the Physical Therapist
• Educate patients and their families on proper care and use of equipment
• Accurately document treatment plans, progress and outcomes in the electronic medical record system
• Communicate effectively with other healthcare professionals and care teams to coordinate patient care
• Participate in quality assurance activities and performance improvement initiatives
  
  
  

Requirements

• Associate's degree from an accredited Physical Therapist Assistant Program
• Current state license as a Physical Therapist Assistant
• Minimum of one year of experience in a home health setting preferred
• Basic computer skills including EMR documentation and Microsoft Office
• Excellent communication and interpersonal skills
• Valid driver's license and reliable transportation
• Ability to pass a background check and drug screening
