We are currently looking for a Home Care Intake Coordinator to join our team. The position would be responsible for providing assessment of patients’ home health care needs and coordination of referrals to home care and hospice from Subacute Rehab Centers within Middlesex County.

Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Coordinates referrals to home care to promote continuity of care
  • Provides individualized counseling to patient and family regarding available home care services
  • Establishes and maintains professional working relationship with hospital staff, physicians, and VNAHG staff
  • Provides back up coverage to assure availability of intake coordinator services
  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications

  • RN; Bachelor’s degree in Nursing preferred
  • Valid NJ nursing license
  • 2+ years of community health nursing experience
  • Excellent verbal and written communication skills
  • Excellent interpersonal and organizational skills
  • Strong client focus, assertive, decision making skills, articulate, and creative in problem solving
  • Strong decision making skills

Working Conditions/Physical Demand : Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license
