Job Title: Healthcare Coordinator/Social Service Specialist
Location: Largo, Pensacola, Panama City, Alachua, Cocoa, Fort Myers, West Palm Beach, Fort Pierce, Tamarac, Miami & Orlando, FL (Multiple Locations) (100% Onsite from Day 1)
Duration 3 Months Contract (with a possible extension)
Job Description
Draws conclusions from reading/reviewing medical records, and then writes corresponding case- notes which highlight the key-points.
Utilizes established guidelines and internal protocol to determine if client meets medical eligibility criteria based on review of medical records and client psychosocial history.
Applies knowledge of aging process, activities for daily living and medical conditions to written case-notes.
Justifies medical eligibility recommendation through written narratives which summarize client’s current situation, limitations, and needs.
Presents case-notes and medical eligibility recommendations to Registered Nurse for approval and adjusts to professional feedback provided by Registered Nurse and/or Supervisor.
Education Requirements
The candidates must meet the minimum qualification of:
A bachelor’s degree or higher from an accredited college or university with a major in a related field, OR professional experience totaling four (4) or more years in a related field OR a combination of education and professional experience totaling four (4) or more years in a related field.
