Responsibilities
Perform comprehensive assessments using our tablet-based assessment tool.
Identify and refer members for necessary case management.
Provide recommendations for follow-up care with primary care physicians.
Conduct assessments in various settings, including homes, clinics, telemedicine, and long-term care facilities.
Travel within designated regions as per licensure.
Execute telehealth assessments with audio and video capabilities.
Maintain an active license in Michigan without prescribing authority changes.
Required Skills And Experience
Minimum of 2 years of experience as a Nurse Practitioner or Physician.
Familiarity with HCC model, HEDIS, and STAR quality measures.
Possession of an unrestricted state license or eligibility for one.
Registration with Medicare and Medicaid.
Board certification in internal medicine, geriatrics, family medicine, or nephrology preferred.
Strong communication skills with elderly and chronically ill individuals.
Proficiency in telehealth technologies.
Ability to effectively use computers, tablets, and mobile devices.
