Submission Requirements

Board Certified Gastroenterology - REQUIRED

Active CO License - REQUIRED or IMLC (LOQ in hand at time of submission)

Active DEA - REQUIRED

Must be available starting June/July - REQUIRED

Must disclose providers specific dates of availability June 28-Sept 30 to be considered - REQUIRED

Must provide # of shifts per month provider can offer to site beyond Sept 30th - REQUIRED

Local/drive-in candidates only - HIGHLY PREFERRED, vendor/provider must take on flight/air travel costs.

No Air Travel - REQUIRED

Gastroenterology Fellowship - REQUIRED

Clean Malpractice/background - REQUIRED - Site requires Self inquiry NPDB report at presentation to be considered.

Must include a copy of IMLC, DEA at time of name clear
