Family Support Specialist / Social Worker

Position Summary

This person will function as a family strengthening team member in coordinating services for children and their families under the care and supervision of the Family Support Supervisor. They will serve as the child and family’s primary advocate facilitating the best possible care and services available.

Role And Responsibilities

This person’s job responsibility includes but are not limited to the following:

  • Conduct intake assessments to determine client’s needs and develop individualized service plans
  • Provide individual and group counseling to clients and their families to address emotional, social, and mental health needs
  • Connect clients to community resources and services such as housing, healthcare, employment, and education
  • Development and maintain relationships with community organizations to facilitate access to resources for clients
  • Monito clients’ progress and evaluate the effectiveness of services plans
  • Advocate for clients’ needs and rights and facilitate communication between clients and service providers
  • Maintain accurate and timely documentation for client interactions, service plans, and progress reports
  • Provide crisis intervention services to client as needed
  • Demonstrate effective communication with supervisors, and team regarding case load, safety concerns
  • Mindfully integrate the Trauma Informed Care Model and Person-Centered Approach into all procedures, practices, and daily interactions
  • Attend individual supervision and staff meetings
  • Provide on-call support in rotation with the clinical team (serve as on-call point person)
  • Follow Florida Child Abuse Reporting Rules
  • Perform all other duties as requested or assigned

  • Strong documentation and record-keeping skills

Physical Requirements

Staff must be physically capable of driving an automobile in a safe manner. Must be able to lift up to 35 pounds, stoop, bend, kneel, stand/walk up to 4 hours, and be capable of continuous activity with children; must be physically able to participate in and complete PCM Practitioner Level 2 certification (Professional Crisis Management).

Qualifications And Education Requirements

  • Minimum bachelors prefer master’s degree in social work, Psychology, Family Therapy, or a relevant field and prefer at least two years direct experience working with children with developmental disabilities
  • Must possess a combination of experience or relevant education, and training which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties
  • Experience with community development practice, crisis, and trauma counseling, working knowledge of systems
  • Experience with developmental disabilities, mental health and illness, behavioral issues, anger management and drug and alcohol issues
  • Ability to take initiative in meeting required contract hours and independently maintain schedule to meet needs of clients and families, in addition to needs of the agency
  • Flexibility of working hours will be required to meet the child and family’s needs which include evening (2-3 per week) and weekend hours
  • Completion of annual 40 hours in-service agency training, including CPR, First Aid and Water Safety certification
  • PCM certification for Practitioner 2
  • Valid Driver’s License and clean driving record
  • Strongly recommend maintaining Bodily Injury Liability insurance of $100,000 per claim, $300,000 per incident on their personal automobile when driving clients or their families
